In the shows pilot episode, which aired in 2001, JD admits that he has feelings for Elliot. In episode three, JD fails to get a kiss however after a romantic moment is interrupted and the 48 hour time that girls go through before "overthinking" begins lapses. This, to JD, causes the two to be stuck in the "buddy zone".
For much of the series initial run, it was a joke among fans and cast that the couple only sleeps together once per season. In season three, when Carla learns the two had sex again, she stated "is it that time of year again".
The two became on and off again couples often during the first half of the series, and the tension continues even up through the latest episode that aired on May 17, 2007. During this episode, when Elliot is about to get married to Keith, and JD's ex-girlfriend is about to deliver his baby, the couple are found in a room together, both trying to escape the serious life changing moments about to occur. The end of the episode finds JD admitting that they are the perfect couple, but just have one major flaw; a fear of committment. The end of the show has them holding hands and leaning in for a kiss.
